Benefits and Provisions of the Dental Membership Plan

  • The enrollment date begins on the first day of the month once the membership fee has been paid in full.
  • The plan is valid for 365 days from the enrollment date.

Membership Plan Includes:

  • 2 regular prophylactic cleanings during the plan year
  • All exams and necessary radiographs
  • A discount on most dental services (excluding Invisalign and retainers)
  • No annual deductibles
  • No annual maximums
  • No preauthorization required
  • No waiting periods or eligibility periods for major treatment
  • No missing tooth clause restrictions or exclusions
  • The annual enrollment fee for each family member is non-refundable.
  • All treatment must be paid in full at the time of service in order to receive membership discounts.
  • Preventive services, including exams, bitewing X-rays, cleanings, and fluoride treatments, must be completed within the membership year and cannot be carried over to the following year.
  • It is the patient’s or parent’s responsibility to schedule and keep appointments for all enrolled family members.
  • This membership plan is non-transferable and cannot be used by another individual or an uncovered family member.
  • Membership benefits cannot be combined with any other dental plan, discount program, or promotional offer.
  • All treatment fees are based on Sera Dental’s Usual, Customary, and Reasonable (UCR) fees, not those of any other dental insurance plan.
  • Membership fees, benefits, and procedures are subject to change at the discretion of the practice.

Exclusions & Limitations

It Cannot Be Used:

  • In conjunction with any other dental insurance plan, discount program, or promotional offer.
  • For treatment that is covered under another dental insurance plan.
  • For treatment that, in the sole judgment of the treating dentist, falls outside the scope of services provided by this practice.
  • At any dental office other than Sera Dental.

Program Guidelines:

  • This plan is only honored at Sera Dental.
  • Membership plans are not dental insurance, but an in-house savings program offered by SERA Dental. By enrolling, patients join a 12-month membership plan that includes preventive care services and discounts on additional treatments. This program allows patients to receive care at reduced fees rather than paying the full standard rates at each visit.
  • Membership dues must be current and paid in full in order to receive membership benefits.
  • It is the patient’s responsibility to schedule and keep all appointments.
  • Any necessary radiographs will be determined by the dentist at the time of service.
  • Membership discounts cannot be combined with any other dental insurance, discount plans, or promotional offers.
  • Membership fees are non-refundable. No refunds will be issued for underutilization of the program, relocation, or if the patient obtains dental insurance after enrollment.
  • This plan is non-transferable. Family members cannot substitute or transfer membership benefits to another individual.
  • Patients receiving orthodontic treatment must remain active members of the plan throughout the duration of treatment in order to maintain applicable discounts.
  • The membership plan is valid for 12 months (365 days) from the date of enrollment. Benefits must be used within the membership year and do not roll over.
  • Membership plans will automatically renew annually unless a cancellation notice is provided prior to the renewal date.
  • Sera Dental reserves the right to terminate membership or refuse treatment if the member fails to pay dues or if the account becomes delinquent.
  • Additional fees may be charged for missed, canceled, or broken appointments without a 48-hour notice.
  • Repeated missed appointments may result in termination of the membership plan.
  • All treatment fees must be paid at the time of service in order to receive the membership discount. Otherwise, standard office fees will apply.
  • We recommend scheduling preventive visits every six months to maintain optimal oral health and to ensure availability.
  • Invisalign and implant services may receive a 15% membership discount.
  • When using CareCredit, Sunbit, or other third-party financing, a 10% membership discount may apply.
